KL Rahul is already not part of the Test squad due to injury, so if Rohit does not play, Mayank and Shubman Gill can open the innings for Team India. The good news for India is that if Rohit's RTPCR test comes back negative, he will not need to be isolated and will be able to play the Birmingham Test.
